Terms of Agreement

As the team captain I have informed my team players of the following: Release of Liability: I agree to the unreserved used of my name and/or likeness (including photographs, videotapes, and other depictions) for publicizing Kemmeter's Corners athletic programs. I assume all risks and hazards of the program and release from responsibility any employee/agent/volunteer organization associated with this activity. In case of injury, I do hereby waive all claims or legal actions, financial or otherwise, against Kemmeter's Corners, its organizers, sponsors, or any volunteer connected with the program unless injury is caused by the sole negligence of Kemmeter's Corners. I also acknowledge that pets and carry-ins are strictly forbidden on the premises and that in the event that any of my team members, including me, fail to abide by this rule, either the aforementioned team member or the entire team may be ejected from a game, season, or tournament without refund at the sole discretion of the management of Kemmeter's Corners. I also agree that, before participating in any way, I and my team members will have read the league or tournament rules in full and agree to abide by them in their entirety. Rules may be found on the Kemmeter's Corners website. In absence of signature, payment and participation in the program shall constitute acceptance of the conditions set forth for any purpose.
